Maybe. However, I had the impression that we talk about different
problems sometimes. The first problem is putting transient windows below
all others in ClickToFocus mode. This can be solved by setting:

FocusOnMapTransient=1
FocusOnMapTransientActive=1
FocusOnAppRaise=1
FocusChangesWorkspace=1

explicitely (tested on my desktop box before it died, and on my laptop I
have no problems whatsover, for unknown reason).

You are right. This is not our problem. All our new windows are created
behind existing ones if FocusOnMap=0.

The other problem appears in focus-follows-mouse-Mode and apparently
affects all windows and not only the transient ones.
I tried to reproduce Wouter's setup (*) and IMO the problem does appear
with the current icewm and disappears with patched packages. You can
find them (for i386) on http://rootfs.net/debs/; aptable, use

deb http://rootfs.net/debs/ ./

We tried icewm-experimental and this gave no solution. The behaviour
is the same as before (with ClickToFocus=0, FocusOnAppRaise=0,
RaiseOnFocus=0,FocusOnMap=0 new windows appear behind existing ones)

We even tried to install the latest icewm (icewm-1.2.30pre1) from
www.icewm.org, but here icewm seems to ignore the (user) settings
ClickToFocus, FocusOnAppRaise, RaiseOnFocus and FocusOnMap.

This is due to new "Focus Settings", set them to "Custom" to make it work like before.
