Using the old poppler library currently in unstable the evince index does not work in PDF Files like the Australian Immunisation Handbook: http://www9.health.gov.au/immhandbook/pdf/handbook.pdf This is now fixed in experimental. Why is is not fixed in etch or sid? This is a serious and old bug long ago fixed upstream.
