I suspect that the problems you are seeing are because of unreadable sectors on the disk. Is there any way to swap or replace the disk and see if the problems go away?

No kernel upgrade since 2006-07-29. Also, until the first problem,
the long self test (Extended offline) didn't fail. It is failing
