While sarge's version (1.5.9-2sarge2) did not exhibit this bug, 1.5.12-1 (from snapshot.debian.net) did. So my conclusion is that it is (some? all?) multipart/alternative messages with no displayable parts that have problems, but not if they're being displayed by themselves.
