Are you sure the specified database file is writeable by the webserver? Is the specified database file valid (perhaps the tools from the db4.4-util package can help here)? At this stage, with the information you've provided this looks very much like a configuration error, or a corrupted database, rather than a bug in PHPwiki. Please provide some more information that would help to pinpoint and reproduce this problem. Particularly answers to the questions above.
