Hi, I have a similar problem, php4 (libapache-mod-php4 4:4.3.10-9) on sid segfaults when accessing mysql, but not all the time.
After a bit of investigation I discovered that libapache-mod-auth-mysql was actually the real problem. In case you have it installed then it is the same bug, if not then disregard my mail. I'm guessing php4 and auth-mysql have not been compiled against the same libmysqlclient, creating some conflicts in the objects. I guess libapache-mod-auth-mysql needs to be recompiled.
