When mysql-server is installed, a special user is provided: 
"debian-sys-maint", that user has all the privileges for creating 
a user.

Its password is located in the file: /etc/mysql/debian.cnf.

It would be a good idea, to me, if phpgroupware uses that user when 
the database choosen is 'MySQL' and the host 'localhost', that would 
avoid 2 debconf screens: no need to ask for a MySQL root name, neither
for the related password.

Some other webtools use such a way to automatically create a MySQL user
(the password could be a random string).
