I think I have some insight into this problem (which I am also having).
'/etc/init.d/bluetooth restart' does not start the passkey agent, so
upon upgrading from bluez-utils 2.x to the new 3.1 version the passkey
agent does not get started. running 'stop' and then 'start' registers
the PINs in /etc/bluetooth/passkey and starts the agent. However, trying
to pair my cell phone (which works in 2.x with bluez-pin) still fails,
using the PIN "1234" /etc/bluetooth/passkey/default and on the phone,
with an "invalid PIN" message on the phone and an I/O error from
hcitool. Just the "no agent registered" message in syslog is gone.
