At the risk of sounding stupid, I have to ask if this whole problem may be
related to having the wrong patch in the  kernel-patch-vserver package:

$ ls -l /usr/src/kernel-patches/diffs/vserver/
total 138
-rw-r--r-- 1 root root 140270 Apr 29 23:44 patch-2.6.16.11-vs2.0.2-rc18.diff.gz
   
but 

$ dpkg -l kernel-patch-vserver linux-source-2.6.15

Why do I think that patch is intended for linux-source-2.6.16?

Also, it looks like Nicolas Ledez tried to use teh 2.6.16 patch without
success.  Maybe this would all go away if we could get a 2.6.16 kernel in
etch (while perhaps keeping 2.6.15 there).
