Yes it's done by the postinst script and works fine here.

Can you do: export DEBIAN_SCRIPT_DEBUG=1
and then reinstall or at least do a "dpkg-reconfigure mysql-server-5.0"
and save the output using script or similar?

The reconfigure does this. Oh and we're talking about a MySQL user not
a UNIX system user, maybe that's a misunderstanding here, too, as you
are asking for "groups".
