Well, just a guess, but perhaps an ugly workaround could be to move the file out of dh_md5sums sight and re-name it back later? Btw... the Debian policy (which is referred by #361038, which, in turn, lead to the exclusion of the README file) doesn't state "you must not" but rather just "you don't need to", i.e. "you are not forced to" :) Exclusion of the original package's README file puts you in the unpleasant position to check it's contents with each new upstream version, to see if it's still candidate for exclusion.
