This bug has been marked as unreproducible for nearly a decade, and without more info on how to reproduce it cant realistically be fixed - i suspect any issue was a local misconfiguration as no-one else has seen this issue.
Anyone watching should try using the '-d' option and it will show more information about which rules are being used.
