I've tried to reproduce this issue on the latest version in Debian (0.4.18-6) and have been unsuccessful. I tried a bunch of things, enabled auto-refresh and even tried to update a PDF generated with pandoc, but I always get a proper focus. I'm on openbox though... I'd be curious to know what Desktop environment you are using, and if you can reproduce this bug in another one.
