   * What led up to the situation?

Inserted a Hauppauge WinTV HVR-950 USB video capture device into a USB
port.

   * What exactly did you do (or not do) that was effective (or
     ineffective)?

Nothing I have tried is effective.  Removing the out-of-tree ZFS modules had no 
effect.  Increasing the amount of RAM installed had no effect.

   * What was the outcome of this action?

Kernel crashed.  Error starts with "usb 1-4: Couldn't create media mixer 
entities. Error: -19".  That is followed by "BUG: kernel NULL pointer 
dereference, address: 0000000000000008" and then a lot more (dmesg capture 
attached below).  Computer appears to still be working after this but it hangs 
when trying to shut down.  It must be manually power cycled.

Not a crash.
