I had a closer look. Upstream did not reimplement the SQLAuthType
Backend, but rather:

Bug 4281 - Redesign support for Backend SQLAuthType for MySQL.  If the
MySQL client library cannot support the SQLAuthType Backend,
mod_sql_mysql will emit a warning on startup.

...removed it.

I applied your patch to 1.3.7rc4 code, I had to fix it; upstream changed
variable names in [1]. The result is here [2]. I did not test the code
and I have doubt, that it works as you expect.

Would be nice, if you could test this. I don't plan to do another upload
of 1.3.6 in unstable, as 1.3.7 will be released soon.
