I saw this some time ago upstream with different printer drivers. for example my kyocera FS1010 works fine with a hp laserjet driver but does not like to be a kyocera. the prin for envelopes. the print was just rotated 90deg. Can you try printing the envelope but with a whole sheet of plain paper (no envelope in the printer) and see the missing parts?
