Hello.

It seems that, rather than "pdf2ps", "ps2pdf" could be the cause
of the problem I'm experiencing.

Here is a specific example.

Step 1:
I scanned a document and saved it in PNG format (this might be
relevant since I think that, at this point, there is no inherent
page size information).

Step 2:
I converted to PS (using imagemagick's "convert").
My printer (automatically) prints this file without any problem.

Step 3:
I convert this file to PDF, using these options to "ps2pdf":
   -dPAPERSIZE=/a4 -dPDFSETTINGS=/printer

Now, I have to use *manual* paper insert for my printer to print
this file!  [And I noticed that the printed contents is shifted
to the top of the page...]

So, is it at all possible that "ps2pdf" inserted some code which
would ask for manual intervention?
