If I understand this correctly, this bug should be critical, not wishlist. A user does the obvious thing of adding an lvmcache in front of their root device, running dpkg-reconfigure initramfs-tools, but then finds their system is unbootable. lvm should either refuse to add a cache in front of the LV used currently as "/" (or there should be a bloody prominent message at the top of lvmcache(7), or adding dm_cache, dm_cache_mq and /usr/sbin/cache_check to the initramfs.
