If ‘monitor-activity’ is off, I get the bell symbol.

Yes.  And actually I have both symbols in Debian stable, where I run
tmux version 2.2-1~bpo8+1 from backports.  Also, I checked with
someone running Debian unstable, and they get also both symbols.

If run tmux with an empty config file and set monitor-activity on, I
get only the activity symbol.
