Dear Maintainer,

        when starting from scratch, MakeGAPDocDoc composes PDF manuals without 
Index in its
        Table of Contents (ToC). If we pdflatexify one morw time, the Index 
appears then in
        the ToC. The test.g GAP script in attachment reproduces the issue.
