I can confirm the issue with the instructions provided by Dan. lintian also sees those errors [1]. That said, this looks like a problem in the xml/xslt toolchain to me. I'm saying this as someone who has no idea about docbook. I'm CCing Zbigniew, maybe he has some ideas. Is this reproducible on fedora as well?
