This always occurs for me when first logging in. It occurs for both SSH and OpenPGP keys. For example, attempting to add an SSH key to the agent with ssh-add results in an SSH_AGENT_FAILURE message (if the pinentry tries to prompt for a passphrase to encrypt the key with). It also prevents signatures with OpenPGP keys. Again, killing the gpg-agent is always effective.
