I had this issue as well. I rebuilt pam-mysql with the very brief patch referenced in http://sourceforge.net/p/pam-mysql/bugs/27/#c2aa and saslauthd hasn't locked up in 2 weeks. Since that link is dated 2009 and the last message asks if the project is even maintained, maybe the patch should be applied in debian instead of waiting for upstream to notice.
