Debconf templates should never make assumptions on the value of debconf widgets which depend on interfaces (dialog uses a Yes/No question for instance while GTK uses a checkbox). I suggest changing this to "Please choose this option if you wish...." Similarly, the short description should be a question (see developer's reference). I suggest adding a question mark at the end.
