The current description of the make-kpkg --initrd option says:

                                       Please note that unless there
    are hook scripts in /etc/kernel or added into  the  hook  script
    parameter  of  /etc/kernel-img.conf,  no  initrd will be created
    (the bundled in example scripts are just examples -- user action
    is required before anything happens).

This is no longer true as initramfs-tools has installed similar
scripts since version 0.94 (and they have respected the INITRD
variable since 0.98).  dracut also does this.
