That looks strange.

The DM_TYPE is set to raid and you don't have the TAGS=:systemd: set.
Do you use some special LVM setup?

Do you have any custom rules in /etc/udev/rules.d/?

Could you list the output of
ls -l /etc/udev/rules.d /lib/udev/rules.d/
