It could be a bug in the Tab Mix Plus extension. It seems that it loses the tabs that are still being loaded. But actually I can reproduce the problem with upstream's Firefox and no extensions, e.g. by typing Ctrl-\ just after starting Firefox to emulate a crash (with browser.sessionstore.restore_on_demand set to false).
