Replying as to how I see this apply to Okular.

In Okular one needs to enable "show forms" before US IRS forms can be filled 
in and checkboxes clicked.  Usually there's a message at the top of the window 
giving the user to enable this option, and it can also be done via

   View -> Show Forms

The good news is that clicking checkboxes work when "show forms" is active.  
The bad news is that the filled-in data doesn't show up in a print; it can be 
saved in the form, but when printing and print-previewing the form via Okular, 
"show forms" seems to be automatically disabled such that X's are shown as 
Check marks and filled-in data doesn't show up at all.

I'll report this problem with Okular separately so that this bug can be closed 
concerning Okular about checkboxes.
