I've had exactly this problem when upgrading from squeeze to wheezy:

Upgrade prompts that my configuration file has changed
I said keep my old one
For some reason this didn't happen and the new config file is in place
History is lost

Since my history goes back to 2004, that's rather more than 50 entries.
