I'm reproducing with two files:
/etc/ferm/ferm.conf:
include 'test' ;
/etc/ferm/defaults:
domain (ip ip6) table filter chain OUTPUT policy ACCEPT;
And yes double-checked that 2.1-4 is actually installed.
## dpkg --list | grep ferm
ii ferm 2.1-4 all
