I have the same issue running wheezy with a recent custom compiled kernel. I also used to have the issue under squeeze ...
root@sake:~ # xm dmesg | grep "Xen version" (XEN) Xen version 4.1.3 (Debian 4.1.3-2) (wa...@debian.org) (gcc version 4.7.1 (Debian 4.7.1-7) ) Fri Sep 7 17:48:19 UTC 2012 root@sake:~ # uname -a Linux sake 3.6.4-00001-g8fb2aff #10 SMP PREEMPT Mon Oct 29 15:27:50 CET 2012 x86_64 GNU/Linux Until "recently" I just worked around the issue by forcing ntp resync even for large offset but recently I got an application that can't tolerate even the small period of time between the unsync and the ntp resync ... It doesn't happen at regular intervals, more like randomly and yes, it cause the offset on dom0 and all domUs running there.
