sorry for not finding the time to look at this earlier. Could you
describe a set of steps how to arrive at this issue, which server is
used and what other xmpp clients show?

I've tried to reproduce this on jabber.fu-berlin.de, by creating a room
with Pidgin, joining with several users one of which uses irssi and the
xmpp plugin, and then typing "/affiliate outcase one-user" in Pidgin. 
In irssi, the message "one-user was kicked from
te...@conference.jabber.fu-berlin.de by
te...@conference.jabber.fu-berlin.de []" is displayed. That's how it's
supposed to be, no?
