It’s harmless, and not shown by the forge unless sysdebug_enabled. I don’t know whether it can occur outside of the FusionForge integration at all. My tracking it down was that something disrupts the matching between MW’s counting of the BEGIN…COMMIT/ROLLBACK blocks and those that actually are sent to PostgreSQL.
