Whenever I see I/O errors with any tool I always check the
/var/log/syslog for any kernel reported device errors, specifically
for disk errors. Usually the tools are just reporting the underlying
problem up the chain. If you are seeing an I/O error then the first
suspect is always a disk drive because they so often fail and cause
that problem.
