I don't know how else to demonstrate.  The overstriking basic pattern
which some manual pages use for bold+underline is _\bC\bC (where C is
the character).

In an xterm or the linux console, running less on the attached
overstrike.txt causes the text to appear as follows:

This works:     bold
As does this:   underline
This breaks:    b^Hbo^Hol^Hld^Hd a^Han^Hnd^Hd 
u^Hun^Hnd^Hde^Her^Hrl^Hli^Hin^Hne^He

--bod
This works:     bboolldd
As does this:   _u_n_d_e_r_l_i_n_e
This breaks:    _bb_oo_ll_dd _aa_nn_dd 
_uu_nn_dd_ee_rr_ll_ii_nn_ee
