1) I did an fsck on all disks, as suggested, just in case.
No disc problems reported.
2) Playing with strace on kpse-which --show-path=ls-R shows that the file
on which it chokes is
/usr/share/texlive/texmf/ls-R
which contains:

which looks odd to me.
It seems to come from texinfo.
/usr/share/texlive/texmf/ls-R
is a link to /var/lib/texmf/ls-R-TEXLIVEMAIN
I will answer the other questions & report further later.
