Yes, it is a known problem. However for fluxbox,
/usr/share/doc/fluxbox/README.Debian read:
   
   Currently, fluxbox supports the debian menu system, -however-, to use
   user-menus, you need to alter the ~/.fluxbox/init file.

and /etc/X11/fluxbox/system.fluxbox-menu read:

# to use your own menu, copy this to ~/.fluxbox/menu, then edit
# ~/.fluxbox/init and change the session.menuFile path to
# ~/.fluxbox/menu

so I would suggest you try to follow these instructions.

If it does not work or if the process should be improved, please issue
a separate bugreport against the fluxbox package.

Yes, it should work. The requirement to use "official" sections is only
meant for official Debian packages. 

If you use GNOME, you might be interested to test the solution I proposed
in <http://lists.debian.org/debian-gtk-gnome/2004/12/msg00046.html>.
