When we encountered that with the teTeX packages, the reason was always
either that we moved or renamed files (like in: old version uses
WMGLOBAL.cfg, new version uses WMGLOBAL, when WMGLOBAL.cfg is present,
preinst renames the file) or that files where moved between binary
packages. The only solution I know of for these cases is to use ucf.

Sometimes it was not easy to reproduce, e.g. some only showed up if some
woody package that we Replace/Conflict/Provide was installed
previously. But when we could reproduce it, the reason became clear soon
- we just had to look for the filename before the upgrade (is it there?
from which package? changed?) and during upgrade. Maybe an md5sum call
in preinst could help, but I never needed this.
