dbmail-smtp is perhaps unable to connect to the database, or it's
connected alright, but the database is locked?

upgrade to 2.0.7. Some serious problems were fixed in the preforking
code since 2.0.4

Next time it happens, try:
#> dbmail-users -l nobody

and if that doesn't work:

#> ps aux

or

#> /etc/init.d/dbmail restart
