See the documentation in the courier-authlib package. Decide how you want to manage your mailboxes: use system accounts, or MySQL, or PostgreSQL, or LDAP.

If you decide that you want to use system accounts (authpam), create a new account using your system's usual tools, create the new account's $HOME/Maildir, and you're done. If you're using one of the other virtual authentication modules, follow the instructions in the corresponding documentation file.

Courier-IMAP uses whatever authentication configuration you've set up using courier-authlib. courier-authlib's documentation includes instructions to verify that password validation works. Do not proceed with Courier-IMAP until you can use authtest succesfully.
