I've never had a ~/Maildir/INBOX folder and have been usingAnd you shouldn't. An IMAP folder "INBOX.x" is not ~/Maildir/INBOX.x
The ultimate troubleshooter is to set IMAPDEBUGFILE=debug.txt in the config file, that creates debug.txt in each Maildir that logs all IMAP commands sent to that account. That's going to generate huge log files fairly quickly, so you don't want to leave it set, for long.
