I always just did it with Qmail (the LDAP patch includes it by
default) and part of the useradd script included sending an email to the
newly created account.  Delivery of the mail made qmail create the
directories.  I do not know if there is such a patch for non-LDAP qmail
but assume it wouldn't be too hard to do.
