THREAT_MODEL: Remove mcumgr leftovers not relevant in core

mcumgr is now part of core, so notes about Zephyr fork, Mynewt only
scope and mapping to former mcumgr repository questions no longer add
value. Remove them and simplify related answers. Question Q20 (scope of
mcumgr model) is removed, other question numbers are kept unchanged as
those are referenced throughout the document.
