Problem Summary:
The flush token back pressure in the load stream was implemented with a
busy-wait loop that polls the number of queued flush tasks and sleeps a few
milliseconds repeatedly until a slot frees up or the timeout elapses. This
wastes CPU cycles and reacts slowly when a slot becomes available.
This PR replaces the polling loop with a condition variable based
back-pressure: the sender blocks until a flush slot is available (or the
timeout is hit), and the slot is released via a deferred cleanup on task
completion, which wakes one waiting sender. This removes the spinning and
reacts promptly to freed slots.
