The FHIR test server is setup to use [DSTU3](https://github.com/apache/camel-quarkus/blob/master/integration-tests/fhir/src/test/java/org/apache/camel/quarkus/component/fhir/it/FhirTestResource.java#L44). You have to change it manually to test out other versions. There is a issue upstream for an [enhancement](https://github.com/hapifhir/hapi-fhir-jpaserver-starter/issues/70) to run the server on multiple versions.
