This is specifically a problem for me when I run icewm, I suspect
that both xfce and kde (if I ever build that again) might start
pulse themselves. So, in the part of ~/.xinitrc where I already
test if I am running icewm, I added
pulseaudio -V --log-target=syslog
(for systemd the target would be journald)
BUT - it failed to get the cookie or to load its modules, and
therefore ended.
From a running Xorg I then removed all of ~/.pulse - some of that
was apparently *really* old, from when I copied things from an old
machine to bring this one up, so it is possible that ~/.pulse is an
old location.
I then successfully started pulseaudio.
Closed Xorg, ran startx again, this time pulseaudio had started and
I had sound in firefox.
