Did you intentionally compare against attlist[$x] for "<!-- start
description" and against $_ for "<!-- start explanation"?
Your while tests
attlist[$x] =~ m|<!-- start description|
and on a false, the || tests
m|<!-- start explanation|
which matches $_, since it isn't explicitly bound to anything with =~
