System log entries around the time of the problems?
Operation not permitted sounds either OS related, or a suboptimal
translation from a SCSI problem to the SCSI API of the OS.
I would make sure the HBA and drive setup can work seamless. RAID
adapters for tape drives, for example, are known to cause trouble sometimes.
