did you have a look on your Apache log files ?
Maybe it can help you ....
Otherwize, did you try to create a dedicated MySQL account with valid
password for bacula-web ?
P.S: the beta version of Bacula-web which is coming soon contain a lot of
improvments, especially for troubleshoot setup issues like you're
encoutering.
This version will be available in the next few weeks.
