Are you sure that your switchtype is correct and your provider has a dms100? If not, change this to national.
If so, the two things I'd try to narrow it down are: 1.) Temporarily remove the AEX410 card and test again 2.) Temporarily disable OSLEC for your echo canceller and test again.
